Methadone products are classified as Schedule II by DEA and therefore require a DEA Form 222. Currently there are two versions of the DEA Form 222 accepted. The proper completion of the newer version DEA Form 222 Single Sheet can be seen here. The proper completion of the previous version DEA Form 222 Triplicate can be seen here. The Triplicate form will be accepted by VistaPharm until 10/30/2021. Once you have completed your DEA Form 222 please mail it to our customer service department at the following address:

This is the same address used on the DEA Form 222 for the supplier information. Order the Methadone 10mg/mL liquid products in quantities of 4 due to cases packaged in these increments as a unit of sale from VistaPharm. When mailing the DEA Form 222, it is recommended to mail utilizing a tracking method of package.
